Does an 2004 E53 3.0 D Sport have a timing belt/chain or is it a gear train system?

I don't think its a serviceable timed/ mileage item. Never heard of anyone changing them. The cam chain followers on the 4.6 is recommended as a precautionary replacement, but the chains are good for ever, or if they get noisy I think.

No you shouldn't have to change it, and it's quite rare for them to cause problems.
